Here are my latest experiences with this scanner, ppSCSI v0.91 and
microtek2 pre-0.8 backend.

When the calibration option is on, as you already know, the scan
doesn't start.

After having removed the scsi_wait_for_image() call in microtek2.c,
the scan starts but the head goes forward/backward again and don't
stop, whatever the 'no-backtrace-option' is set or not, and with all
the different scan modes (lineart, gray or color).

Thus, the best way to use this scanner is the default one, with
the 'no-backtrace-option' set and the 'calibration-by-backend' option
unset.
In this case, everything seems to work correctly, but the offset
problem remains :-(
