You are not using the current version of SANE/Microtek backend.
Get SANE-0.72 (which has v0.6 of the backend) and see what happens.
(My guess is that you have version 0.4 or 0.5 of the backend.)

The "RealScan" is really a ScanMaker E2 --- which for some reason declares
itself to have 1-pass capabilities, while it is a 3-pass scanner. Current
versions of the backend (starting with v0.6) fix this.

If it still doesn't work with the newer software, send a debug log (follow
the directions on the sane-microtek man-page), and indicate which versions
of the software, OS, scsi card, etc, you have (it can make a difference).
